Brad Caldwell: Transforming Ophthalmic Healthcare Technology with Customer-Centric Leadership

Healthcare technology is evolving at an unprecedented pace, but for Brad Caldwell, meaningful innovation begins with people. As Chief Executive Officer of Optivate, he’s built his leadership philosophy around delivering exceptional customer experiences, improving patient outcomes, and creating a workplace where employees can thrive both professionally and personally.

With decades of experience spanning post-acute care, home health, hospice, and healthcare technology, coupled with personal experiences that reinforced the transformative power of medical innovation, Brad leads with resilience, purpose, and a steadfast commitment to putting patients first. In this interview, he reflects on the experiences that shaped his journey, the leadership principles that guide his decisions, and his vision for the future of healthcare.

From Adversity to Purpose-Driven Leadership

The last twenty years of Brad’s professional life have been spent in post-acute care, home health, and hospice – fields where “technology matters more than almost anywhere else, because you’re serving elderly people aging in place, often with very little family support nearby.”

The moment that made it personal for him was when he faced a health scare and underwent six operations to save his eyesight. “For the first time, technology and healthcare weren’t abstract to me – they saved my life. I was scared I wouldn’t see my kids grow up. Ophthalmology carries some of the most advanced technology of any medical specialty, and what it gives people in return is nothing less than the freedom of sight,” he observes.

Brad’s most pivotal career moment that shaped his journey to becoming CEO of Optivate began during his college career. As team captain for two years, he led a team that won just two games in total. That challenging experience taught him resilience, leadership, and perseverance – qualities that would later define his success as a CEO. “Leading people is easy when you’re winning, and things are going your way. Leading through difficulty is what actually shapes you as a leader, and that lesson has stayed with me ever since,” he reflects.

Success Through People, Purpose, and Culture

Brad sees success as a journey. “I want it to change my employees’ lives for the better. I’ve told our sales team directly: give me a year, and I’ll change your life. We can teach you to become the best version of yourself while you’re heavily compensated for growth. When the company wins, we all win,” he says.

Balancing the demands of leading a tech-driven healthcare company with personal priorities has been one of the hardest parts of Brad’s role, but it’s also a principle deeply embedded in the company’s culture. “We treat our employees’ families as part of the team. I don’t want a single person on our team to miss a kid’s sporting event or school play because of work,” he insists.

As a highly remote workforce that’s also metrics-driven, he maintains that those two things aren’t in conflict. “Being remote and being disciplined about what we measure is exactly what makes it possible to protect people’s time with their families,” he affirms.

Brad’s leadership style has always been rooted in a customer-first approach – happy customers make new customers. “Nothing energizes me more than hearing a genuine reference from a customer or getting a referral from a friend of our business,” he says.

Most of his professional career has been in high-growth, high-intensity businesses and M&A. “Shifting a culture to become fast-paced and metrics-driven takes time, and isn’t for everyone – and that’s okay. Stepping into my current role was exciting precisely because Optivate is so customer-focused already,” he states.

As of now, his team is learning to manage the pace of the growth that they’re seeing, and his favorite part of the job is watching people get excited once it clicks for them. “At that point, the job stops being just a job and starts feeling like a movement. It’s genuinely fun to be part of something that’s changing healthcare. My number one goal is to make Optivate the best customer experience company in the world,” he declares.

Customer-First Innovation Driving Healthcare Excellence

Optivate has been recognized as best-in-KLASS for customer experience for years running. For Brad, that’s more than an award – it reflects a company culture where the customer comes first, even ahead of revenue. “Do the right thing, and the dollars follow,” he affirms.

He admits that working within the U.S. healthcare system doesn’t always make sense, especially when it comes to billing, as you have to make sure you follow the process carefully. “As U.S. healthcare slowly shifts toward a value-based model, the focus has to be on the whole patient, treating every person with genuine care,” he reflects.

In his efforts to foster a culture of innovation and adaptability at Optivate, Brad believes in trying new things constantly and being willing to fail fast when something doesn’t work. “Innovation comes down to trusting your gut and taking the risk. Challenge the status quo, and believe in your instincts,” he remarks.

While he doesn’t love leading with AI, Brad admits that, at this point in the industry, “you almost have to.” For him, AI isn’t about replacing people – it’s about making all of us better at the skilled parts of our jobs, especially for clinical staff. “AI is making our doctors better and freeing them to focus on what actually matters – the patient,” he notes.

Leadership Shaped by Mentorship and Purpose

Brad feels fortunate to have had some great leaders and mentors along the way, noting that he’s also worked for a few poor ones who focused only on the bottom line. “That contrast shaped me. I’m a true believer that if you run a healthy, customer-focused business, the revenue follows,” he observes.

He hopes to leave a legacy that focuses on patient experience and clinical outcomes – because when you get those right, the proper financial results follow. “I remind our team every week that we’re impacting thousands of people’s lives, every single day. Our customers are restoring or improving people’s eyesight. It’s hard to overstate how much that can change someone’s entire outlook on life,” he says.

Brad’s advice to aspiring leaders who want to make an impact in healthcare technology is that it all comes down to effort and passion. “Believe in your passion and in yourself, and good things follow. Don’t be afraid to fail – just fail fast and correct course quickly,” he recommends.

Conclusion

Brad Caldwell’s journey is a testament to the power of purpose-driven leadership. From overcoming personal challenges to championing customer-first innovation and cultivating a people-centric culture, he’s consistently demonstrated that lasting success is built on trust, resilience, and meaningful impact.

As Optivate continues to redefine healthcare through technology and exceptional service, Brad remains committed to empowering his team, enhancing patient care, and embracing innovation that strengthens – not replaces – the human connection at the heart of healthcare. His vision serves as a compelling reminder that when people come first, sustainable growth and meaningful transformation naturally follow.
